The service

EMR Chart provides charting and clinic operations software delivered over the internet. EMR Chart is a record-keeping and workflow tool. It does not practice medicine, does not provide clinical advice, and never replaces the professional judgement of a licensed clinician.

Accounts

Accounts are individual and must not be shared. You are responsible for keeping your credentials and multi-factor device secure, for the activity that occurs under your account, and for telling us promptly if you believe an account has been compromised. Practice administrators are responsible for granting and removing access for their own workforce.

Acceptable use

• Do not access records you have no care or business need to see.
• Do not attempt to bypass authentication, authorization, rate limits, or audit logging.
• Do not upload malicious code, or use the service to send unsolicited bulk messages.
• Do not probe or load-test the service without our written permission. Vulnerability reports are welcome through our contact page.
• Do not use the service unlawfully or to infringe others’ rights.

Patient data and the practice agreement

A practice that stores patient information in EMR Chart does so under a written service agreement, including a business associate agreement where required. That agreement, not this page, governs data ownership, permitted uses, breach obligations, retention, fees, term, and governing law. Where this page conflicts with an executed agreement, the agreement controls.

Availability and changes

We work to keep the service available and perform maintenance in a way that limits disruption, but we do not promise uninterrupted operation on this page. Features may change as the product develops; we will not remove a capability a practice depends on without reasonable notice.

Suspension

We may suspend an account to stop active harm, such as a suspected compromise, misuse of patient data, or abuse of our email or infrastructure. Where practical we contact the practice’s administrators first.

No warranty of outcomes

The service is provided as it is. Clinical, billing, and compliance decisions remain the responsibility of the practice and its clinicians. Using EMR Chart does not by itself make a practice compliant with any law or standard.
